Home is the best place to provide patient with physical, psychological, and soul healing.  When your family member can not take care of you, what should you do?

What is the service for the preparation of discharge:

personnel, patient, and family members to utilize the medical and social resources to let the patient and the family members to receive proper continuing cares for improving the patient’s health and also learn the related health education knowledge with ability to take care of oneself.  For the continuing cares after the discharge, the hospital will offer resources to the patient and the family members to ensure the patient will receive the continuous and proper cares.
 
Object of the service:

The hospital will offer all the patients with the service for the preparation of discharge.  Different diseases will require different course of services; therefore the following patients are for the first priority:
  1. long term in using the tubes such as for the foley, NG feeding, tracheotomy which will require long term cares and home cares services.
  2. Have been hospitalized for long period of time which can not return home yet.
  3. Diabetics require health education consultants.
  4. Patients require referral service to nursing homes or other non-acute institutions.
  5. Patients require using medical equipments.
Services Items:
  1. Education on the cares: provide related pamphlets, brochures, and video tapes.
  2. Provide information or referral: home cares, chronic ward, nursing home, senior citizen homes, and application for the disability subsidy.
  3. Assisting to purchase or rental the related medical equipments: wheelchair, crutches, bed pan, oxygen generating machine, and sucking machine.
  4. Free consultant.
