After the physician confirmed that you will need to admit into hospital, please take the ‘admission notification’ issued by the physician to the admission center locate at first floor of the medical building for registration. (After office hours, please go to emergency center.) Office hours for the admission center:
Mon. – Fri. 8:30 to 18:00
Sat. and Sun. 8:30 to 12:30
Fill in the ‘Consent Form for Admission’.
Please bring the following items with you when you are admitting into the hospital:
NHI card
ID, driver license, or household registration for proof of ID.
Patient with ‘Occupational Hazard Application’, please hand it to the ward secretary in order to take care about the waiver of partial charges.
Patient with catastrophe diseases please informs the attending physician during the course of hospitalization so that the physician will screen to see if the patient qualifies for applying the waiver of partial charges.
